Yes, the buyer does have to wait for you to approve.
Once the offer is approved or countered (then the buyer has to approve) then the buyer will have to return to complete (pay) checkout.
At the bottom of the listing there is an instant offer price
If you put the minimum price in this box (please include any shipping also), supposedly if the offer is above this price, then the sale will go through without you having to approve.
I have this filled out on mine but so far I have had to approve all offers.

Why would the instant offer price have to include s&h?
Say the item price is $100 with $10 S&H or $110 delivered. If you want the instant offer price to be $60 ($50 + $10 S&H), you are saying to put $60 as the instant offer price and not $50 that will automatically add the $10 for s&h???
Also, if you have set the “instant” offer price, why would you have to manually accept it? Isn’t that the whole point of “instant”, that it accepts it automatically?

Remember these tips:
- Use links to other sources to support your opinions
- Use examples where possible
- Put yourself in the inquirers shoes: what extra info would be helpful?
Should I post a comment or an answer?
You can only post one answer, so make it count. Maybe your reply is more fitting as a comment instead?
Post an answer for:
- Replies that directly and specifically answer the original question
Post a comment for:
- "Thanks," "Me too," "I agree," or "Works for me" types of replies
- When you would like the original poster to provide more details
- When you have more to add to someone else's question or answer
