Questions about how global shipping works

I am considering using global shipping, but there are still some things I don't understand. If you have several different orders that would be shipping on the same day, do you put them all together in one package/box with each package individually...

What is included in free shipping to the us?

Does that include Hawaii, Alaska, Guam, Puerto Rico, Virgin Islands? It's hard to include those area in the free shipping price and still be fair to those in the original 48. Else Hawaii and Alaska will ponce on my cheap items. If it were all USPS...
